Part A

Assignment Context and Scenario LO1
LO1: Examine the health, care and support services available to an individual
requiring multidisciplinary care
Scenario
You are working in a community-based healthcare service. You are designated by the
team manager to research the health and social services available within your area to
meet the needs of an individual who is now requiring multi-disciplinary care.
Activity 1
For the assessment of LO1 you are required to write a report of no more than 1,500
words containing evidence of your research and findings. Your report should consider the
services available within your local area and how they meet the needs of an individual
requiring multidisciplinary care.
You will need to:
• Outline local community resources and provision that supports integrated care
planning/working
• Describe local unmet need related to healthcare provision in own locale
• Explain the difference between health and social care providers and types of
interagency care provision in relation to meeting the needs of the individual
requiring care
• Evaluate local resources/provision in terms of meeting the needs of an identified
individual requiring multidisciplinary care
Your report will be a summary of your research into the local health and social care
provision in your area, within which the organisation you are in placement with operates.
You will be expected to include referenced images, and a map of your local area to
identify the services discussed.

Part B
Submission of Part B should be in the form of a portfolio of evidence of practice based
reflection.
LO2 Assess an individual’s capacity to identify their own needs
For the assessment of LO2, you will be required to produce two elements:
1. an anonymised care assessment (filled out by you)
2. reflect on your anonymised care assessment
Assignment Context and Scenario (LO2)
Scenario
You are required to complete a work placement in a healthcare setting. Whilst you are
working you will need to develop and complete a case study on an individual who has a
person-centred plan to meet their health and social care needs, in addition to this you will
need to reflect on your anonymised care assessment and own practice in supporting
different individuals and your team when providing care support.
For the assessment of LO2 you will be required to produce two elements:
1. An anonymised care assessment which demonstrates the capacity of an individual
to make choices in their care pathway. This will be the start of your case study.
2. A report of no more than 1,500 words regarding the differences in care
assessment processes.
You will need to:
• Outline an individual’s capacity to support the identification of their own care
needs
• Describe your role and that of different healthcare professionals in supporting the
individual through person-centred care
• Explain the process of different person-centred assessments in defining the
individual’s own care pathway
• Critically evaluate a capacity assessment of an individual’s care needs to identify
areas for improvement
Guidance on the Anonymised Care assessment (Care Plan)
1. The anonymised Care Plan should be submitted as a word processed document
2. The layout should include the care plan and an analysis of the care worker and
the individual’s role in the development and review of the Care plan (see example
at the end of the assignment brief of a Health Care Plan)
3. Reference the analysis with reference to research using the Harvard referencing
system
4. The recommended length of the case study is 1000 words.

anonymised care assessment and work practice, you will include observations of your:
1. Partnership working within a multidisciplinary team
2. Skills required for person-centred communication
In your reflection, you will need to:
• Identify the relationships and partnership working within multidisciplinary working
within multi-disciplinary teams
• Describe the responsibilities of information sharing between multidisciplinary
teams
• Analyse the advantages and disadvantages of information sharing within the
multidisciplinary team and the impact on the individuals care needs
• Provide a detailed analysis of your role in partnership working and coproduction
with the individual for the delivery of the care pathway
• Describe the responsibilities involved in balancing individuals’ rights and choices
with duty of care
• Explain the differing communication methods used to support an individual in
meeting and reviewing their care needs
• Analyse a balanced approach to positive risk taking in supporting the individual to
meet their ongoing care needs
• Critically evaluate your role in facilitating and empowering the individual to
communicate their changing care needs
Guidance for the Observation Reports (See template at the end of the assignment
brief)
1. The observation reports should be used by your supervisor to record your
performance and to what standard (See observation report at the end). The student
will complete the observation report and the workplace supervisor will check it and
sign it.
2. There will be a section for your work place supervisor to make notes. The observation
write up form will be filled out by you and signed by your work place supervisor.
3. The tutor will discuss with you which Assessment Criteria you have successfully
achieved and give you feedback. You will receive feedback on the standard the
standard formative feedback form.

Guide to student
1. Preparation guidelines of the Coursework Document
a. All coursework must be word processed.
b. Any assignments submitted with TEXT BOXES will not be accepted.
c. Document margins must not be more than 2.54 cm (1 inch) or less than 1.9cm (3/4
inch).
d. Font size must be within the range of 10 point to 14 points including the headings and
body text (preferred font size is 11).
e. Standard and commonly used type face, such as Arial and Times New Roman, should
be used.
f. All figures, graphs and tables must be numbered.
g. Material taken from external sources must be properly referred and cited within the
text using Harvard system
h. Do not use Wikipedia as a reference.
2. Plagiarism and Collusion
Any act of plagiarism or collusion will be seriously dealt with according to the College
regulations. In this context the definitions and scope of plagiarism and collusion are
presented below:
Plagiarism is presenting somebody else’s work as your own. It includes copying
information directly from the Web or books without referencing the material; submitting
joint coursework as an individual effort.
Collusion is copying another student’s coursework; stealing coursework from another
student and submitting it as your own work.

Suspected plagiarism or collusion will be investigated and if found to have occurred will
be dealt with according to the College procedure (For details on Plagiarism &
Collusion please see the Student Handbook).
3. Submission
a. Initial submission of coursework to the tutors is compulsory in each unit of the course.
b. The student must check their assignments on ICON VLE with plagiarism software
Turnitin to make sure the similarity index for their assignment stays within the College
approved level. A student can check the similarity index of their assignment up to five
times in the Draft Assignment submission point located in the home page of the
ICON VLE.
c. All Final coursework must be submitted to the Final submission point into the Unit
(not to the Tutor). The student would be allowed to submit only once and that is
the final submission.
d. Any computer files generated such as program code (software), graphic files that form
part of the coursework must be submitted as an attachment to the assignment with
all documentation.
e. Any portfolio for a Unit must be submitted as an attachment in the assignment
4. Good practice
a. Make backup of your work in different media (hard disk, memory stick, etc.) to avoid
distress due to loss or damage of your original copy.
5. Extension and Late Submission
a. If you need an extension for a valid reason, you must request one using an
Exceptional Extenuating Circumstances (EEC) form available from the Examination
Office and ICON VLE. Please note that the tutors do not have the authority to extend
the coursework deadlines and therefore do not ask them to award a coursework
extension. The completed form must be accompanied by evidence such as a
medical certificate in the event of you being sick, and should be submitted to the
Examination Office.
b. Late submission will be accepted and marked according to the College procedure.
It should be noted that late submission may not be graded for Merit and Distinction.
c. All late coursework must be submitted to the Late submission point into the unit
(not to the Tutor) in the ICON VLE. A student is allowed to submit only once and
that is also treated as the final submission.
d. If you fail in the Final or Late submission, you can resubmit in the
Resubmission window.
