Assessment Task – Case Management Portfolio

Length: Equivalent of 2000 words (broken down to approx. 1500 words of written justification plus supporting documents)

Task explanation: Using the same case study that you used for your research report, you are to:

Develop a case management portfolio to demonstrate how you will approach the case management process from start to end.
Your portfolio should ‘tell the story’ of the work you’ve done and clearly show how you have progressed from one stage to the next.
Each entry of your case portfolio should document your ‘work’ with the new referral as you progress through the stages of engagement, assessment, intervention, and termination.

Your Case Management Portfolio must include:

Sample assessment(s)
Example case records to demonstrate your ‘work’.
A sample intervention plan/case plan (you may use the proforma provided or develop one of your own)

You should include written justification of why you chose to work with the client in this way. This should include a discussion of the theoretical framework/practice approach you have applied and some evidence as to why this is the most appropriate model to use in this situation.

You should also include any other relevant considerations which have influenced your practice (such as ethics, policy, practice context, and/or legislation).

Your Case Management Portfolio may:

Include further supporting documentation if you wish. Such as:

sample case notes
sample referral letters
sample intake forms
