This assignment gives you the opportunity to combine all of those skills and apply them to a case study specific to your field of practice.

The assignment is to be submitted via Turnitin and should be 3000-words (+10% maximum).

In addition to reviewing the information below, you will find it useful to look at the slides and/or recording from the Assessment Launch lecture. There is some supplementary information about the ADPIE nursing process in the VLE folder in the Learning Materials section of the module shell that may also be helpful.

Recommended structure

IntroductionInform the reader of the aims and prepare them for the content of the assignment. Introduce the chosen case study from the module family.
Keep this brief and try to avoid repeating everything that is in the profile. Try to imagine they are a real person and only focus on the issues relevant to the assignment.It may be useful to briefly note the importance of the nursing process, assessment and care planning within the specific field of nursing practice you are studying and link this to the NMC code (NMC,2018).Assessment and DiagnosisIdentification of the needs from the case study (assessment).Detail a holistic assessment of the persons needs (more than just physical – can include healthcare inequalities, health promotion elements, needs related to wider determinants of health etc.).Opportunity to include elements of nursing theory/approaches (e.g. Maslow’s hierarchy of needs, Activities of Daily living).Can consider how needs would be assessed with relevant nursing assessment tools. What do these needs mean for the individual (diagnosis)? What impact are they having?Choose one need to focus on for the rest of the assignment – this can be one particular need or theme of needs (e.g. uncontrolled epilepsy, diabetes, an element of health promotion). Justify this focus – consider wider determinants, prevalence/impact, justify through nursing theory/approaches.   Summary: by the end of this section, you need to be clear on what needs the person has, which one you have chosen to focus on and why.PlanningHow will you address the persons identified need?Identify potential interventions and plan for them (potential to discuss why these elements are important – what is their impact, are they best practice etc.).Consider any relevant goals/outcomes for the persons need.Determine your priorities for the chosen person – can you offer a justification for these?Would it be appropriate to involve anyone else in the planning process? Other family members? Other professionals?   Summary: by the end of this section you need to have presented your plan for meeting the needs you have chosen to focus on.ImplementationHow will the plan be implemented? How will this be documented? Who will carry out the interventions?Discuss the evidence-based nursing interventions that will help to resolve the chosen problem or unmet need.Can include elements of person-centred care if relevant and/or consideration of wider determinants (e.g. is the individual likely to have a health promotion needs or experience a healthcare inequality, will this impact implementation?)Consideration of relevant wider determinants for that individual (e.g. involvement of the family)You may wish to refer to elements of nursing theory and best practice guidelines (e.g. NICE guidelines) to justify your decision making.   Summary: by the end of this section you need to have discussed how your plan will be fulfilled.EvaluationHow will you know the implementation of the plan has been successful?Link evaluative elements back to interventions within the plan and implementationConsider the why, how, when and who of evaluationDiscuss the evaluation process in terms of methods and frequencies. How would the intervention be evaluated?You may wish to refer to elements of nursing theory in this section too to justify the importance and process of evaluation – refer back to the cyclical nature of ADPIE.Consider how your evaluation will be achieved and what the outcome of it may be.   Summary: by the end of this section you need to have discussed how your plan will be fulfilled.ConclusionThis is a summary of the main points of your assignment. Make sure you include the patient in this final section – refer back to your intended outcome/goal. Avoid bringing in any new material.
