A 73 year old Asian man presented at hospital after suffering generalised weakness, dizziness and palpitations for two weeks. He had experienced such symptoms at frequent intervals since adolescence. He had previously been hospitalised on multiple occasions for fever, chills, rigours and cough. His family had no history of consanguinity.

Respiratory and cardiovascular assessments were normal. He displayed a distended abdomen and his liver and spleen were enlarged and palpable. Laboratory investigations revealed the following results:
A peripheral blood smear demonstrated microcytic RBCs with anisopoikilocytosis, target cells, teardrop cells, few microspherocytes and schistocytes.
