Mrs Judth Anderson is a 65-year-old lady who lives alone since her husband passed away 5 years ago. She has two daughters who live nearby and visit regularly. Yesterday afternoon, Mrs Anderson tripped over her walking frame at home and was unable to get up. She spent four hours on the floor until her neighbour Tom, who heard Mrs Anderson calls for help and went to investigate. Tom called the ambulance and Mrs Anderson was brought to the Emergency department (ED). After being examined by the ED physician, Mrs Anderson was transferred to the medical ward for haemodynamic management and monitoring of her kidney function.

Background

Mrs Anderson has a history of Atrial Fibrillation (AF), Hypertension, Non- Insulin Dependent Diabetes Mellitus (NIDDM), and early-stage of Chronic Kidney Disease (CKD). She has regularly seen by her GP to monitor her renal function. Her NIDDM was diagnosed 2 years ago and recently required a dosage change to insulin injection only.

Situation

You are a nurse working in the medical unit. After receiving the handover from the admitting nurse, you read through the ED admission note including the blood pathology report and medication list. A few medications are due to be administered to Mrs Anderson in the next hour.

Assignment Task and Instruction

Apply the clinical reasoning cycle to commence your nursing care for Mrs Anderson regarding safe medication delivery and your nursing considerations when administering her oral, sublingual, subcutaneous and intravenous medications. Create a POWERPOINT presentation and record a 10-minute presentation to demonstrate how you ensure a safe medication delivery is implemented, and the quality of nursing care regarding medication administration is delivered for Mrs Anderson

Tips

1. Use headings to divide each clinical reasoning cycle – i.e. one heading per step per slide. Your thought process needs to be congruent and logical.

2. A good use of the Learning Pharmacology Table to help you describe each medication and synthesise the medications with the patient’s condition. Your Learning Pharmacology table can be presented in a relevant step in one slide.

3. A good rationale linked to the patient’s conditions on each clinical reasoning cycle is needed. (Note: the medications used in this assignment should reflect the knowledge you have learned from Module 1 to Module 6).

4. Your presentation must not exceed 10 slides, word count to fit into each slide (you can present each step in the clinical reasoning cycle on each slide,
