IMPORTANT
You may be required to and are permitted to research and reference your responses
You may use the learner guide, internet research, recommended texts and or
websites/information and/or other reliable resources.
You must reference your source where required.
Case Study | Post Traumatic Stress Disorder (PTSD)
You have been working as an emergency responder for the past five (5) years. Six (6) months
ago you and your new partner responded to a multiple casualty MVA.
A family of five (5); a mother, father, nine (9) month old baby boy and their grandparents were
all injured in a car accident.
On arrival the scene was chaotic, and there was only one policeman in attendance. Witnesses to
the crash were all trying to help the injured family; and on reflection you felt that it took longer
than usual to start emergency treatment on the patients because of this.
Both the grandparents passed away instantly during impact, and the baby boy suffered a
cardiac arrest on scene and could not be resuscitated. Both parents critically injured, were
treated on scene and transferred to the nearest trauma hospital, where the father later passed
away too.
You participated in a debrief shortly after the incident, however you felt that this was not very
useful for you in dealing with the horror of what you witnessed on that day.
You have spoken to your supervisor and a workmate on a couple of occasions but not told them
the depth of your distress. You have had some time off to ‘try to get your head straight’ but this
has not really helped.
You have been experiencing vicarious stress since attending the scene, and this is impacting
your wellbeing.

AT1.2 Case study and Response
Since the MVA you have continued working however recognise you are experiencing some
serious stress responses and have noticed you are impatient with others, both socially and at
work.
You feel you are revisiting the trauma over and over, and it has impacted on your sleeping
resulting in nightmares, You have suffered significant weight loss due to loss of appetite,
drinking and have taken up smoking again after 5 years, you feel anxious all the time, frustrated,
disempowered in your work and have significant guilt issues due to your believing you were
unable to save the victims at the scene, especially the baby.
You have always loved your job but now you feel like you can no longer be an effective
paramedical clinician.
