THE ASSIGNMENT BEGINS HERE: Write a detailed and comprehensive Intervention Plan for a client with whom you are currently working or with whom you have worked in the recent past. You are not required to implement this Intervention Plan. Follow the guidelines below to write your plan. PART A: Begin your plan with the following background information: ⦁ Client profile: provide a client description, including age, sex, ethnicity, abilities, challenges, family situation, social network, any other relevant client characteristics. (Make sure that the presenting problem is worded objectively and observably. For example: Bob scratches the skin off his arms when a demand is placed on him.)….no more than 2 sentences ⦁ Provide a baseline documenting the presenting problem. The baseline will include the following: ⦁ The context: setting(s), people present, time(s) of day, activities. ⦁ Data documenting what you observed (all relevant verbal/nonverbal behavior’s). This is your basline how are you proving your problem exists ⦁ A brief analysis of the possible reasons for why the behavior(s) may have occurred. ⦁ Mission and/or mandate of the institution or organization. ⦁ Legal framework: laws, legislation, policies or regulations related to the client, client group, institution and/or organization. PART B: Next, design your Intervention Plan, including the following information: ⦁ A statement about the priority need of your client. Ensure this priority need comes from one or more of the possible reasons you identified in the baseline you provided. ⦁ A long-term goal statement: a sentence indicating the overall, (long-term) intervention goal that addresses the priority need of the client. ⦁ Two short-term SMART intervention objectives that directly address the goal and that will help your client achieve the goal. Explain why these objectives will help to meet the priority need identified for this client. ⦁ The methodology (including a detailed description of the time frame, environment, activities, tools, other participants) for achieving each of the two objectives. ⦁ Evaluation: Explain how you will evaluate your intervention plan. Discuss the appropriateness of your methodology for achieving the objectives. What specific factors might affect the achievement of the objectives? Note: You may develop your intervention plan following the guidelines provided in this Evaluation Booklet. You may also use an intervention plan template previously devised and used by a Special Care Counsellor in your organization, provided that all required elements are included.
