PROJECT PROPOSAL
PROJECT NAME- LIFE STORY
PROJECT MANAGER- SOCIAL WORK STUDENT (JEMI JOHNSON)
DESCRIPTION OF PROJECT– Life Story Project aims at providing a holistic view of older adults, promoting them relationship centered care, enhancing person centred care and improving their quality of life (Wills, & Day, 2008) making them feel special and helping them to memorise their past events by making Life Story books on them involving collection and recording aspects of both person’s past and present life. This Life Story Book also helps care givers to communicate and engage with the residents living with dementia and understand their unique needs by providing them meaningful interaction based on the resident’s likings and disliking’s and their personal history.
To start with this project, Social Work Students first need to take the consent from the resident in a written form confirming that they are willing to share their personal history. It involves approximately 3 to 5 sessions of 45 minutes to 1 hour interviewing resident informally to know and record their stories depending upon the resident sharing the story on how much they are willing to share. It includes writing the story, taking the pictures of resident’s important events, achievements and memorable days. On being completed, it gets printed in a form of book and handed over to the resident.
WHY WAS PROJECT INITIATED?
A life story book is a wonderful way to bring back memories for a person living with dementia, and at the same time provide useful information to caregivers about that person.
The Life Story Project was initiated: –
To support resident’s social interaction.
To calm the resident being overwhelmed by a new environment.
To help the resident transfer their focus on positive memories.
To support the resident’s mental stimulation.
To gain better understanding between the resident and their caregivers.
Life Story Book is a quick reference guide for care givers as they give insight into the person’s individual preferences and historical events that may have an impact on their reactions to certain stimuli and situations.
AIMS/ EXPECTED OUTCOMES

To reduce their emotions of depression, loss and grief by making them read their positive memories, achievements and important life events.
